If you have Type 2 Diabetes, you may be eligible for a Type 2 Diabetes Care Plan through your GP, providing Medicare-subsidised access to up to 9 sessions with an Exercise Physiologist, Diabetes Educator, or Dietitian. 

These include: 

  • 1 assessment for group services per calendar year 
  • Up to 8 group allied health sessions per calendar year 

You can also combine a Type 2 Diabetes Group Allied Health Services Referral with a Chronic Disease Management Plan, increasing the number of subsidised sessions available to support your long-term diabetes management.
